Third Party Administration (TPA) Access

Complete a copy of the IL Power of Attorney (POA) form required by the Illinois Department of Employment Security. 

  • Use this document for guidance on filling out this form. 
  • Add the following information regarding the Third-Party Agent lines: 

When you have that form filled out and signed, you have two options: 

Option A 

  1. Log in to https://mytax.illinois.gov/_/. 
  2. Select Online Upload. 
  3. Select Full Access to view and manage 
    • UI Employer Contribution Report 
    • Rate Notice 

 Option B 

  1. Return the signed POA form to Payroll Vault Franchising Payroll at pvtax@payrollvault.com. 
  2. Payroll Vault Franchising will request TPA access by uploading the signed POA form to the agency website.
